Planning for Adulthood:

Secondary transition planning is a process that develops each year to prepare high school students for life after high school.  Goals evolve and change as the student grows through experiences and gains new insights.  The items below are a guide to assist with the process of preparing your child to be as prepared as possible for adulthood.

Financial and Legal Support Services

As students near the age of majority at the age of 18 financial and legal aspects will become their responsibility unless guardianship is in place.  It is important as students move from entitlement of the public school system to eligibility qualifications with community agencies to know what is available.  The Financial and Legal Supports Services domain is intended to be a guide of resources for students and families as they navigate the financial and legal aspects of adulthood. 

Individuals with and without disabilities will be faced with many financial decisions and legal responsibilities.  Some of the life events may include creating a will, navigating and maintaining eligibility of SSI or SSDI, guardianship, saving and spending money, or accessing the Developmental Disability Supports Waiver.  Each service may require an application to determine eligibility and could have a waitlist of months to years.  It is important to start the process early.

It is important to know which agency is available to assist with each unique situation you may encounter.  Building a relationship with the agency personal can be key in navigating the requirements and benefitting from the wide range of supports they can provide.  The financial and legal areas can be intimidating with the technical terms and processes and it is extremely important to reach out to professionals that can assist with the process.  Centrum for Disability Services is a non-profit agency that assists individuals with anything SSI or SSDI related, from what is the purpose to how to apply and continued eligibility.  Using the resources provided as well as communicating with professionals will make the process of accessing services easier for you and your loved ones.

Employment

The Employment domain is a guide of resources for students and families as they navigate the supports available for individuals leaving public school and moving toward eligibility of services.  The resources offer a variety of services for individuals with and without disabilities from competitive employment, supported employment, and job training.  Employment supports are intended to assist individuals to maintain employment, gain skills, and support job accommodations.

Working is a natural component of adult life.  Research shows that individuals who have work experience in high school leads to adult employment for student with disabilities.  Providing high expectations proper support, and a variety of opportunities to learn vocational skills will lead to jobs and careers.  When individuals have an opportunity to join the workforce they build confidence and a network of contacts.

Competitive employment is when individuals with and without disabilities work together and have comparable wages.  Supportive employment is individualized support for the individual to maintain a job and support accommodations.  Anyone can access the Wyoming Workforce Services for employment supports.  Workforce Services may determine an individual needs more supports and refer them to Vocational Rehabilitation.  Using the resources provided as well as communicating with professionals will make the process of accessing the services easier for you and your loved ones.

Extensive Supports and Developmental Disabilities

Extensive Supports and Developmental Disabilities domain is a guide of resources for students and families as they navigate the supports available for individuals leaving public school and moving toward eligibility of services.  The agencies in this domain are for individuals that qualify as having a developmental disability or individuals with significant disabilities that require lifetime supports.  The supports are intended to provide independence and quality of life. 

The majority of resources in this domain are located in Laramie Wyoming or within the state of Wyoming.   Extensive supports can provide case management, personal care services, respite, skilled nursing, transportation, supported employment, community support, and community living services.  Each service may require an application to determine eligibility and could have a waitlist of months to years.  It is important to start the process early to have supports in place when an individual is moving into adulthood. 

Families and individuals with significant disabilities require care and support to navigate their day to day life and activities.  It is important that these individuals receive the proper services to reach their personal goals, work on independent living skills, and to continue to build self-determination skills. The University of Wyoming has a variety of resources such as the Speech and Hearing Clinic.  The University of Wyoming Disability Institute (WIND) has the Family Support Network and Assistive Technology Resource Center. Using the resources provided as well as communicating with professionals will make the process of accessing services easier for you and your loved ones.

Independent Living & Advocacy

Independent Living and Advocacy domain is a guide of resources for students and families as they navigate the supports available for individuals leaving public school and moving toward eligibility of services.  The agencies in this domain are for individuals that may require more supports for independently living in their community as well as skills training, peer support, and advocacy supports. 

The majority of resources in this domain are located in Laramie Wyoming or within the state of Wyoming.  Independent Living and Advocacy supports can provide housing assistance, family mentors, disability rights and responsibilities, and access to community resources.  All individuals have the desire to live, work, and to be socially connected to their community with additional supports they have opportunities to be contributing members to their community.  Independent living supports are intended to lead to independence with community living.  Providing advocacy supports, training, and skill building will provide the individual with an understanding of their rights and responsibilities as well as who to go to when they need assistance. 

Each service may require an application to determine eligibility and could have a waitlists of months to a year.  It is important to start the process early to have supports in place when an individual is moving into adulthood.  The Parent Information Center is a good resource to reach out to when looking for information on advocacy and independent living.  Using the resources provided as well as communicating with professionals will make the process of accessing services easier for you and your loved ones.

Mental Health & Other Related Health Needs

Mental Health and Other Related Health Needs domain is a guide of resources for students and families as they navigate the supports available for individuals leaving public school and moving toward eligibility of services.  The agencies in this domain are for individuals with and without disabilities that need access to mental health and affordable health care.

The majority of resources in this domain are located in Laramie Wyoming or within the state of Wyoming.   Agencies in this domain can provide social emotional support, health related education, and a variety of health services.  Mental health is a serious illness that affects 1 in 5 adults and 1 in 6 youth aged 6-17.  Finding the right supports to help someone with mental illness is crucial in providing the proper supports, strategies, and accommodations.  Uplift, NAMI Wyoming, and Peak Wellness can provide mental health supports for all individuals.  Starting the search for a mental health provider that fits the needs of the individual can be a lengthy process as it takes time to build the trust and relationships.  Starting early to find a mental health provider can be key in making the transition to community agency support.

Community Living & Participation

Community Living and Participation domain is a guide of resources for students and families as they navigate the supports available for individuals leaving public school and moving toward eligibility of services.  The agencies in this domain are for individuals with and without disabilities to have the supports and services in the areas of transportation, post-secondary learning, and other areas that lead to being contributors and members of the community.

The majority of resources in this domain are located in Laramie Wyoming or within the state of Wyoming.   Agencies in this domain can provide access and mobility to transportation options as well as post-secondary learning opportunities.  Each service may require an application to determine eligibility and therefore it is important to start the process early to have supports in place when an individual is moving into adulthood. 

Community access is important to all young adults.   Access to transportation and post-secondary education allows these individuals to reach their personal goals and to continue to build self-determination skills.  Frontier Access and Mobility provides access to adaptive transportation to individuals and their families.  The University of Wyoming, Laramie County Community College, and WyoTech provide degrees and certificates and offer supports through their disability support centers.
